A quick and easy way to smooth skin, while restoring and improving detail, is to use a method called Frequency separation. The adjustments will all depend on the quality of the original image, so create smart objects so you can adjust later.
Duplicate your image (CTRL/CMD + J) twice, naming the first duplicate 'low frequency' and the top layer 'high frequency'. Right click on each of the two new layers, and select 'Convert to Smart Object'

With the Low frequency layer selected, add enough gaussian blur (Filter -> Blur -> Gaussian Blur) to remove any imperfections in the skin.

Now select the High Frequency layer, change the layers blend mode to Linear Light. Add a High Pass filter (Filter -> Other -> High Pass). Adjust the slider so you have nice sharp lines, but not enough to blur the detail.

Add a Layer Mask to the High Frequency layer. Select your brush tool (B), with a low opacity/flow rate. Make sure your brush is set to black. Now gently brush over the skin, bringing in the smooth layer below.

Finally clear any skin blemishes with the Healing Brush Tool. Create a new layer, calling it Blemish Removal. Select the Healing Brush Tool (J) and touch up the skin.
Because we created Smart Objects, you can now go back and adjust the sharpness (High Frequency) and softness (Low Frequency) by double clicking on High Pass or Gaussian Blur in your layers window.
