It'll take some time, but I can certainly smooth most of the pixels out for you JR.
Let me know if you still intend to use the image, and I can give it a go for you.
It's just a case of blowing it up huge and smoothing the various tones out on a new layer using my tablet.

maybe your image improvement psd changing some of the settings followed by some exposure adjustments?
